THIS FORM IS FOR ANYONE SELLING FOOD ITEMS. THIS INCLUDES PACKAGED COMMERCIAL FOOD.
**Food booths must complete Health Department form from the Yuba County Environmental Health office and return it to us with this application.**

Please Do Not Forget the Following Items
- This application completed and signed. Deadline: August 1, 2025 or until spots are filled.
- Your payment for the food space of the amount indicated above made payable/sent to ALMA FESTIVALS ENTERTAINMENT, LLC upon receipt of application.
*Applications without the payment will not be processed. - > Cash, Check, Zelle, Apple Pay, Venmo, Cash App, and Money Orders are all accepted forms of payment <
- A complete listing of any and all menu items you wish to sell, including drink sizes and pricing.
- Logo, slogan, pictures, videos, and social media handles for Instagram, Facebook, TikTok, and website emailed.
- A copy of your valid CA Seller’s Permit, health permit, and food handler’s (ServSafe) certificate. Food Handler’s certification is required by the State of California.
- Liability insurance is required. Email CERTIFICATE OF INSURANCE and ENDORSEMENT form with this application.
